Thank you for accepting your student’s seat at CCSC! We are thrilled to have your family join the CCSC Community.
The next step in our registration process is completing a few additional forms online and uploading several required documents. When you are ready to complete these forms, please click here. Note: you also will receive an email with this information and a link to the registration packet at the email address you provided when accepting your student’s seat.
You will need several documents and pieces of information to complete the forms. We recommend gathering these items before beginning this final stage of the registration process:
- Official document with student’s date of birth (birth certificate, medical record, guardian affidavit, etc.)
- Caregiver ID (driver’s license, state issued ID, passport, etc.)
- Proof of residency (dated within 60 days)
- Student’s school transcript (high school) or their most recent report card (middle school)
- Immunization record
- Most recent physical
- Health insurance information
- Name and contact details for your student’s doctor
